Position Summary:

The MDS Nurse Coordinator is responsible for the timely completion of accurate assessments and interdisciplinary care plans that meet Federal and State guidelines. Ths position is a min of 20 hours weekly but less than 32 weekly.

Job Responsibilities

  • Identify resident acuity and needs, help to determine specific care needs and communicate needs / expectations to families and responsible parties
  • Complete assessments, PDPM and care plans for all residents assigned
  • Monitor completion of PDPM's by other disciplines within time frames prescribed by regulatory guidelines
  • Advise supervisor of incomplete and/or untimely assessments by disciplines other than nursing
  • Ensure accurate, timely completion of the Triggers sheet for assigned residents
  • Initiate care plans and supporting activities that will result in best possible outcome for assigned residents
  • Monitor and ensure completion of 7-day look-back documentation by nursing staff within prescribed time frame
  • Maintain current knowledge of PDPM assessment process
  • Run the Restorative program in the community
  • Responsible for Quality Measure Program

 Qualification

·         Licensed RN with the Iowa state

·         2 years MDS and Care plan experience

·         1 year previous experience in a long term care environment

·         Knowledge of Medicare MDS and PDPM

·         Experience in Restorative Programming, Fall care, monitoring and investigation prevention

·         Knowledge of Medicare and Medicaid reimbursement
